Exit Questionnaire

Greetings, ASU Student Athlete:

I am reaching out to you to conduct your Faculty Athletics Representative (FAR) exit interviews -- part I (digital questionnaire) and part II (oral exit interview). This will give me a listening opportunity as you share of your student-athlete experience as I seek to ensure student-athlete well-being. Again, here are the steps:

Part I: Complete this digital questionnaire: [Once you complete this online form, please contact me (jigwe@alcorn.edu) so that we can arrange a face-to-face (preferable) or telephonic meeting.] Once you have submitted be sure to e-mail me to inform me that you are finished. Then, we'll conduct part II (the oral exit interview).

Part II: The FAR (Dr. Igwebuike jigwe@alcorn.edu) will contact you for a follow-up interview. Once this part (II) is finished, I will, as appropriate, inform the Department of Intercollegiate Athletics a'la Mr. Cable (senior compliance officer) to effectuate your release.

Please know: As a critical personnel outside the athletics department, I wish to effectuate this critical conversation towards achieving Alcorn's mission-critical student athlete well-being, compliance, student academic performance, academic integrity, and, ultimately, institutional control function. The President of Alcorn State University is unstinting in his requirement that all student-athletes have exit interviews. Indeed, as your FAR, I look forward to this student-athlete engagement opportunity.
