By filling out this form you agree to:
  • Treat each athlete, opposing coach, official, parent, and program coordinator with dignity and respect.
  • Any person demonstrating abusive, offensive, or foul language or behavior will be asked to leave the facility for a minimum of the remainder of the game and up to the rest of the season
  • Be a role model for the players by displaying good sportsmanship, appropriate conflict resolution, and effective communication.
  • Do my best to learn the fundamental skills, teaching, evaluation techniques, and strategies of my sport.
  • Notify Club staff immediately if a youth discloses information about any abuse, violence, or neglect they may be subject to, or if I suspect a youth may be in danger of harm.
  • Commit to support the mission of Boys & Girls Clubs of Greater Salt Lake.
  • Follow the organization's policies and procedures.
  • Arrive for my scheduled games and practices on time or otherwise give at least a day notice to the Athletic Director if I am unable to coach.
  • Hold in strictest confidence and not disclose, use, copy, publish, summarize, or remove from the Clubs' offices or any Club facility, any confidential* information
  • Submit all required documents including volunteer agreement, volunteer application, background check, and Prevent Child Abuse certificate of completion.

  • Boys & Girls Clubs of Greater Salt Lake understands and agrees to:

    • Supervise, train, and provide resources to volunteers for their positions.
    • Give volunteer assignments compatible with skills and interests.
    • Provide volunteer with feedback on their performance.
    • Inform volunteer of the organization's rules and policies.
    • Treat volunteer as part of the team.
    • Communicate any changes in schedule and information on upcoming events.

    This is an honorable agreement, rather than a legal one, and may be cancelled at any time by either party. This agreement is not regarded by either party as an employment relationship.

    *Confidential information includes, but is not limited to, personal information regarding Club members, clients, employees, officers, or board members, or any research, business plans, marketing plans, products, services, supplies, marketing, finances (including financial statements), or other business information of the Club.

We are thrilled to have you join us as a coach! Youth safety is our number one priority, therefore, we screen all volunteers through a comprehensive screening process. Below is an overview of our volunteer process that must be completed by all volunteers.

1)    Complete the Preventing Child Abuse Online Course for Adults Working with Youth. All volunteers must send a certificate of completion to smccollum@gslclubs.org.

2)    Pass a criminal background check. Volunteers are required to Submit a Background Check online. You will need to create an account and input your information.

Once again, thank you for creating a safe environment for youth in the community. Without great volunteers like you our program would not be possible.
