This data will be shared back with you once all responses are submitted and the data are organized. With this survey, we hope to better understand the ways in which Oregon Veggie Rx programs compare to one another in terms of functionality, funding, populations served, etc. With this information, we hope to identify areas for future standardization, and through that, ways to develop a state-wide expansion.

All questions are optional, but strongly encouraged. You may include as much/little detail in your responses as you’d like, and can use either bulleted or sentence form. If you do not know the answer to a question, please indicate that in your response or leave the question blank. Please note that “you” is often used in this survey, and refers to the program you represent, rather than you as an individual. 

It may take awhile to complete this survey (there are 7 sections), but please note that Google Forms automatically saves your responses as you complete the form. If you have an internet connection, you may exit from the survey mid-way through and return later on to complete it.
