Do you agree with this statement?
I agree that I am able to provide my own transportation to the GO CAPS satellite classrooms as well as business partner locations via my own car, public transportation, carpooling, or parent/guardian. I have considered gas expenses and additional mileage on my auto. *
Do you agree with this statement?
I understand that my business partners and high school team members depend on my commitment to the GO CAPS program; therefore, I commit to the program for a year. If you plan to graduate at semester, you do not qualify for GO CAPS. *

Students accepted to Medicine and Health Care will be required to provide proof of immunizations (additional vaccines above the requirements for public school enrollment are necessary), receive the annual flu vaccine, complete the Missouri Family Care Registry ($14.25 fee), submit to a drug screen and complete a background check. No religious or medical exemptions will be accepted by the hospitals. Please see our website or your school nurse for a complete list of required vaccines and meet with your school nurse to discuss what you are in need of. I understand that my application is not complete until all documentation has been received by GO CAPS.
