Fill out and submit this form to register as a vendor for the Holiday Market held Saturday, December 3, 2016
10:00am to 4:00pm at the White Clover Grange.
The White Clover Grange is located at 36585 Highway 53, about 2 miles east of U.S. Highway 101
No hot prepared food vendors will be accepted for this event since the White Clover Grange will be hosting a hot lunch fundraiser in the downstairs kitchen.
You will be responsible for setting up your own table. Vendor set-up starts at 8:00am day of event.
$30 PAYMENT DUE by Friday November 18th to reserve your vendor table.

Items for sale: List anything different than what you had for sale at the Manzanita Farmers Market during the summer of 2016 (you do not need to list items already approved at 2016 summer market). If you were not a vendor at any of the 2016 summer markets please list here what you will have at your table.
Vendor table: choose your table option. Regardless of your choice here you will have 8 linear feet of space for your table display.
