Periscope: Artist Entrepreneur Training 2020 Application
Thank you for your interest in Periscope: Artist Entrepreneur Training.

To be eligible, applicants must be:
- practicing artists who are currently working professionally and/or are looking to make their artistic and creative endeavors into a sustainable venture;
- not currently enrolled in a degree-granting program; and
- residents of Tennessee.

For this application, you will need to complete all of the required questions, as well as submit work samples (see instructions below) and your résumé or CV (3 pages max). This form may not be saved and returned to at a later date, so we suggest reviewing the entire application before you begin, and drafting your submission in a separate document.

All application materials are due no later than Thursday, February 13 at Noon. Applications received after this time may not be considered.

Periscope participants are expected to complete post-workshop surveys each week of the curriculum, as well as pre- and post-evaluations for the program. Alumni are requested to complete annual evaluations for at least two years after completion of the program.
ATTACHMENTS (Work samples and résumé/CV)
Once you complete the online application, please submit your attachments (résumé and work samples) to the Arts & Business Council. PLEASE NOTE THAT YOUR APPLICATION IS NOT COMPLETE UNTIL YOUR RESUME AND WORK SAMPLES HAVE BEEN RECEIVED.

1) Guidelines for Work Samples:
**Submit the type of work sample most relevant to your artistic genre. For example, a musician will need to submit only audio or video samples; a visual artist will need to submit image samples, etc. Please use your best judgement.**
• Audio or Video: Select up to 2 audio or video samples that best represent your work. Samples may not exceed 5 minutes. (Include Title or Description, Medium, Length of the Entire Work, and Date of Completion or Performance) Files may be submitted as MP3s (audio), MP4s (video), or links provided.
• Images: Select 4–6 images that best represent your work. (Include for each, the Title or Description, Medium, Size (L x W x H), and Date of Completion.) Files must be submitted in JPG or PDF format.
• Manuscripts: Select up to 2 writing samples that best represent your work. Samples may not exceed 5 pages each. (Include Title of Work, Chapter/Section, Date of Completion, Date Published, Publishing House if applicable). Manuscripts must be submitted in Word or PDF format.

2) Guidelines for résumé/CV:
• 3 pages max.
• Files must be in Word or PDF format.

How to Submit Attachments:
