As of Friday, May 13, 2022, the undersigned, parent(s) or guardian(s):

 1. certify as follows:

a.     the undersigned entered into a VTT Enrollment Contract with VTTA for the 2022-23 school year (the “Electronic Enrollment Contract”).

b.     the undersigned have made, or caused to be made, such examinations or investigations as are, in the undersigned’s opinion, necessary to make the statements contained in this Certificate and the undersigned have furnished this Certificate with the intent that it may be relied upon by VTTA in connection with the Electronic Enrollment Contract and VTTA’s assessment of the undersigned’s eligibility for reduced tuition in accordance with the VTTA’s AP policy.

The undersigned acknowledges and agrees to provide to VTTA the undersigned’s 2021 Notice of Assessment issued by the Canada Revenue Agency upon the undersigned’s receipt of same and in no event later than June 30, 2022 along with such other documentation reasonably requested by VTTA (the “Supporting Documentation”) to confirm the information provided in this Certificate: *
If the undersigned does not provide the Supporting Documentation within 10 business days of VTTA’s request, or the Supporting Documentation indicates that any statement made in this Certificate is inaccurate, as determined by VTTA in its sole discretion, VTTA may not grant reduced tuition under the AP and may require that the undersigned immediately pay the full tuition amount set out in the fee schedule attached to the Electronic Enrollment Contract.  If the undersigned fail to immediately pay the full tuition amount, VTTA may relinquish the space held for the child(ren) of the undersigned subject to the Electronic Enrollment Contract for the 2022/23 school year and offer such space(s) to another applicant. *

The undersigned make this solemn declaration, conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath and by virtue of the Evidence Act (Canada).
