The following form must be completed on a weekly basis in order to inform staff of an absence from training or fixtures. Please only complete the following form if you will be absent from training.

Players are expected at all training sessions and fixtures (Tuesday, Thursday, Saturday and Sunday, + additional fixtures where applicable) as agreed to within the EFL Code of Conduct upon registration. Any reason for absence must be provided below as well as via informing a Lead Phase Coach.
