MBA D1 Summer Registration Form/Waiver
Welcome to our 2nd Annual D1 Summer League! Each team will play one game per week with a Round Robin Tournament to close out the season.  5 weeks total.

There will be two divisions. Boys entering grades 3-5 next school year will play on Tuesdays (July 1-July 29), those entering grades 6-8 will play on Wednesdays (July 2-July 30). Game times will be at 4:00, 5:00, or 6:00 pm at St. Pat's in Chatham, NJ. 

Jerseys will be ordered immediately. The jersey fee is included in the registration fee of $265. Payment via venmo @coachmchale or zelle: 8593537149 will complete your registration. First-come, first-served...spots are limited and will fill fast! Teams will be formed after all spots are filled and game times for the season will be announced via TeamSnap two weeks prior to season start.

Consent Agreement - I the parent or guardian of the child or children/adults contained in this registration hereby give my approval to his/her participation in any and all McHale Basketball Academy activities during the current season. I assume all risks and hazards incidental to such participation including transportation to and from activities: and I do hereby waive, release, absolve, indemnify, and agree to hold harmless Dan McHale Camps & Consulting, LLC, McHale Basketball Academy, St. Patrick Parish (Chatham, NJ), Corpus Christi Parish (Chatham, NJ) its officers, sponsors, supervisors, participants, and others whosoever, are associated with my child's participation in these activities, and persons transporting my child or children to or from these activities for any claim arising out of any injury to my child.
