Course Change Request Form SEMESTER TWO CHANGES ONLY
Student email is the preferred email. Please use this if at all possible and make sure you are checking it frequently.
Please be aware that not all changes can be accommodated. We will do the best we can. Please also note, if we are able to fulfill your request you will see the changes immediately in the student portal. Please check the portal regularly to look for updates - counsellors will not be sending email confirmation unless clarification is needed.
Don't forget - we share online course offerings with boards from ALL over Ontario - if you want a specific course and you are open to taking it online, let us know - we can request online offerings as well to provide flexibility in your schedule and to help you gain experience and capacity in the online delivery model. If a course you have requested is full, would you like to go on the waitlist? Should a spot open up, we will add you to the course right away and let you know via your student email.
The parent/guardian authorizes this change for the above named student (please type parent/guardian name as signature)
A copy of your responses will be emailed to the address you provided.
