My child is a student in *
I am familiar with what Positive Behavior Intervention and Support (PBIS) is at Salem Middle School (i.e., expectations, rules for different areas of the school, PRIDE acronym, and rewards for appropriate behavior) *
I have been made aware of the Salem Middle PRIDE acronym used to teach and model behavior expectations (P:Positive Words & Actions; R: Responsibility for Self; I:Integrity and Honor; D: Display Self-Control; E: Excellence in Everything). *
My child has received behavioral acknowledgements or reinforcements (i.e., phone call home, Student of the Week, email, post card, etc.) for following and demonstrating the school's PBIS expectations since this school year started. *
My child has received behavioral intervention (phone call, e-mail, letter...etc) for failing to demonstrate the school's PBIS expectations since this school year started. *
I believe the PRIDE program at Salem Middle School encourages positive behavior. *
My child feels safe at school. *
The expectations for student conduct at SMS are fair. *
Do you believe your child knows how to implement PRIDE (Positive Words and Actions, Responsibility for self, etc)?
Do you use PRIDE at home to reinforce positive behavior?
What suggestions do you have to improve the Positive Behavior program at Salem Middle School as we plan for next year?
