No Cost Electronic ID (EID) application form
If you would like to receive no-cost EID, please fill out this application form to place an order for tags. We provide 840 button tags, white tags to producers and orange (OCV) tags and white tags to veterinarians. 


Note: The tags are No-Cost, but you will have to pay for shipping to get them to you. Please call 303-869-9230 to pay for the shipping costs for these tags. If you do not pay these charges your tags will NOT be shipped.
