Legal Support for GoPuff Workers  
This form is for GoPuff workers who participated in the November 23, 2021 strike and were retaliated against. If you believe you were retaliated against and are interested in legal representation, please fill out this form.  

Examples of possible retaliation include:
- being terminated
- being deactivated
- suddenly being unable to login to the app to drive
- no longer receiving shifts, or
- being targeted with negative comments by supervisors due to your participation in the strike.

Form responses will be confidential and accessible only by Fair Work Center and Towards Justice, two legal organizations that are available to provide no-cost representation GoPuff workers who have experienced retaliation.
