Grievance and Discipline Appeal Information Form
All information pertaining to this appeal must be completed and received no later than 45 days prior to the start of the General Convention. Petitioner will submit an original and ten (10) copies of the appeal. Faxed or  email copies will be accepted, but the original  and ten (10) copies of the appeal along with supporting documentation must immediately follow to arrive within five (5) days after the email or faxed copy. All entries and supporting documentation must be typewritten or legibly printed in black ink.   Failure to submit all information in a timely manner will result in the petition not being considered by the Grievance and Discipline Committee. Petitioner should appear in person before the Grievance and Discipline Committee. If unable to do so, then an advocate, who is a Brother in Good Standing, may represent the petitioner. The petitioner will also state, in writing, the reasons for his not being able to appear in person.
