However if a parent/carer is struggling to complete the form, schools can complete the online form on their behalf.
Alternatively, schools can complete the form below and the information entered on this form will be used to assess the child(ren) for a FSM.
Before completing this form, you must ensure you have:- checked the Synergy SAM 'FSM Eligibility' report to see if an application has already been made.
- the parents full name, date of birth and National Insurance (NI) or National Asylum Support Service (NASS) number.
- the parent/carers email address and contact phone number, which will be used if we need additional information.
- their consent to request an assessment on their behalf.
