Please complete this form for each child in quarantine. Once you submit this form, a district Nutrition Services staff member will reach out to you to confirm pick up day and time. If you need delivery, or your student has a food allergy, please contact our Nutrition Services department once you have submitted the form. We can be reached at: 952-556-6150 or Nutrition@District112.org. The specific meals we send home will be based on current supply and inventory.
