Healthy Black Pregnancies- Integrated Care for High Risk Pregnancies (ICHRP)
Our mission is to have ALL Black babies be born healthy, on time, and at full weight. As a community, ICHRP wants to partner and journey with your family through pregnancy, birth, and the initial postpartum years. We offer culturally responsive broad-based family-oriented services (such as community outreach, Drop-in sista-circles, storytelling, empowerment groups, health & wellness screening, collaborative care planning, referrals, follow-up, and supportive father programs/services). We also assist with coordinated, integrated care (such as health, housing, social services, and chemical dependency in a culturally conscious, family-oriented process).
Complete this form, and someone from ICHRP will contact you within 3-5 business days. So please reach back; we look forward to getting to know you soon.
