Carnegie Connection Birthday Commercial
This form is to be completed at least 5 full school days before the commercial is to be aired. Commercials that are submitted the same week as they are requested may not be completed in time. Commercials will be aired every Friday the week of the birthday. We ask for a minimum donation of $20 for a birthday commercial (if you are able to donate more we always appreciate it).
