PHS Enhanced Program Application 2020/21
Both of the forms below must be completed for application to the Enhanced program. Students are required to complete their Self-Assessment and their Portfolio. A Grade 8 teacher must complete the Teacher Reference form online.
Application Checklist
A completed application will include:
Part A: Student Self Assessment
Part B: Student Portfolio
Part C: Copy of Final Grade 7 Report Card
Part D: Grade 8 Teacher Reference (completed and submitted by teacher)

Completed applications must be submitted by the due date. Failure to do so may jeopardize your acceptance into the program. Students are responsible for making sure their teacher is aware of the deadline for submission of Part D.

Only candidates accepted into the program will be contacted by the Selection Committee. Please be aware that there may be additional costs incurred by students/parents for learning opportunities outside the classroom when involved in the Enhanced Program. These activities are unique to the Enhanced Program and may require supplementary funding.
