YIS Document Request Form
Yokohama International School issues the following documents for former students and alumni.

1. Official High School Transcript - sent only directly to universities*
2. Certificate of Graduation
3. Unofficial Certificate of Final IB Results**
4. Letters of Recommendation - sent only directly to universities
5. YIS School Profile

* An unofficial copy can be issued directly to the student.
** Official IB results can only be sent directly from the IBO at the request of the individual who earned them.

Please fill this form to make your request. We will prepare and send your documents within a week. Please be aware that this may take longer if requested during a school holiday. You will receive an email when the documents have been sent.
