Uniform Order Form 2018 - 2019
* This Uniform Order Form is only available from March 5 - March 9, 2018.
* Please fill in all personal information completely and select the items that you want to order.
* If the item you need is not listed, please contact the uniform committee.
* Please do the payment after receiving the confirmation (invoice) by email once you submit the order.
* Payment can only be done via transfer to SMAK PENABUR Kelapa Gading BCA account 2883206363, the latest by 9 March 2018. Please write the invoice number and student's name on the transfer note.
* Please print the invoice twice and submit them together with the payment receipt to the committee the latest by 9 March 2018.
* The uniform committee will take one of the invoices and stamp the other one. The stamped invoice will be given to the students/parents for collection of the uniform (the exact date will be informed later on).
