Please complete this form if you would like to order virtual meals for your child for the month of April 2021.
Complete a separate for EACH child in your home.

This form is for TREMPEALEAU ELEMENTARY ONLY.  Look for other school forms if you have a child in another building.

• "Virtual meals" are meals offered to students on school days when they are learning from home.
• If a student signs up for virtual meals, they will receive BREAKFAST AND LUNCH for that day.
• Each virtual meal pack will include 1 white and 1 chocolate milk and all of the meal components for breakfast and lunch according to the menu (subject to change).
• 5-day a week virtual students will need to pick up their virtual meals daily at their respective schools each morning, prior to the start of school.  Friday virtual meals can be picked up on Thursday morning with the Thursday meal.
• For all face-to-face students, Friday virtual meals will be sent home with students on Thursdays at the end of the day.

This form needs to be completed and returned to school by April 4th, 2021.
