Instructions
Please Read prior to completing this application form:
  1. Associations that have two (2) or more teams in one Division can apply for a select team in that age group.
  2. Associations that have two (2) or less teams in one Division may collaborate with Neighboring Associations to provide a Select Team for all players within the specified Associations.
  3. All Collaborative Select Programs must be approved by the NOHA prior to being able to host tryouts.
  4. Collaborative Select Programs must be hosted by one of the participating Associations. This Association will be responsible for the registration of this team on the HCR and apply for any Tournaments/Exhibition Games on their behalf.
  5. The Team Fee for Select Teams is $60/Team
