Before filling in this form, please be aware of the following:

- We reserve the right to refuse bookings for parties such as those for 18th/21st birthdays.
- You must only be in the building for the hours that you have paid for, because 1) we may have booked in other hirers before/after you and 2) you are only insured for the hours you have been invoiced for.
- If your function will include the sale of alcohol (including the sale of tickets that provide a ‘free’ drink), you must complete a Supplementary Licensing Form. Please see the licensing form for more information.

There is a space at the end of the form if you feel you wish to add any information in relation to your booking.

Hiring discounts - do you qualify? *
We offer a reduced rate to residents of Wheathampstead (AL4 8) and regular hirers (after an agreed period of time). Please confirm which category you fall into so we can invoice you accordingly. If you are unsure which box to tick, please contact us on 01582 832541.

Will alcohol be sold? This includes the sale of tickets that provide a ‘free’ drink. If you answer YES to this question, you must complete and return a supplementary licensing form.

By ticking the box below, you are confirming that 1) you have READ AND ACCEPTED the Conditions of Letting (found on our website), 2) you agree to pay a 20% deposit within 7 days of receipt of the first invoice, plus a refundable damages deposit of £50 or £100 (see section 4.0 of the Conditions of Letting), 3) you agree to pay the outstanding balance within 30 days of receipt of the final invoice, 4) you agree to complete a supplementary licensing form if alcohol is to be sold during the event and 5) you are over 18 years of age.
