2019 Teresa Favuzzi Advocate of the Year Submission Form
Starting this year at the 2019 Independent Living Conference, California Foundation for Independent Living Centers (CFILC)'s will recognize one individual and / or organization for outstanding advocacy efforts. CFILC encourages you to nominate someone among our membership who you believe is deserving of this award.

Teresa M. S. Favuzzi was a lifelong Disability advocate and Executive Director of CFILC who co-founded Disability Capitol Action Day. To friends and colleagues in the national Disability Rights movement, "Fearless Favu" was an intrepid leader who made the fight for justice, equality and inclusion her life's focus. SELECTION CRITERIA
An accomplished, dedicated California advocate who:
• Significantly contributes to advancing disability rights and positively changing systems
• Is committed to increasing equality
• Is viewed by others in the community as a leader
• Has expanded opportunities for people with disabilities to live independently

SELECTION PROCESS
CFILC will solicit written nominations from individuals and organizations across California. Disability organizational leaders are strongly encouraged to solicit input from their colleagues and peers when making a nomination. To the greatest degree possible, the selection process should be free of actual or perceived conflicts of interest. One award winner will be selected.

RECOGNITION
The Teresa Favuzzi Advocate of the Year will be presented to the winner at CFILC’s 2019 Independent Living Conference on Wednesday, October 9, 2019.
