This survey is not anonymous; your @berkeley.edu address will be recorded, and you will be asked to enter your student ID number. Additional personal information is optional. Aside from your "final remark," responses to this survey will not affect your grade in the course in any way.

Many questions are optional. It's fine if you don't answer the optional questions (though we certainly appreciate it if you do).

Access to your responses will be tightly controlled. The only person who will see this information unanonymized is Jedi Tsang. We will not look at any individual responses except the "final remark" until final grades are submitted. We will only use your email address and student ID in order to measure broad statistical relationships between student scores and survey responses, so that we can better understand the effectiveness of course components.

Aggregate distributional information about all responses may be shared with the course staff, other members of the Berkeley community, or in external presentations. Your individual privacy will be closely guarded in these cases.
