I’m Upon submission, I hereby waive, release, and discharge any and all claims for damages, personal injury, and/or property damages that may subsequently occur to me and/or my participating dependent(s) as a direct and/or indirect result of participation in said programs.

This waiver, release, and discharge is a prerequisite for participating in any Youth of Ummah (YOU) programs. In the event of such damage, whether due to negligence on the part of the aforementioned or otherwise, this release is intended to relieve YOU and its parent organization (IONA), as well as their respective authorities, employees, volunteers, and agents, from liability in advance. It is acknowledged that some leisure activities have a degree of risk or the potential for accidents, and I hereby assume these risks with full knowledge of them. Furthermore, it is acknowledged and agreed that this waiver, release, and assumption of risk applies to my heirs and assignees in addition to myself. 

Minors - To the best of my knowledge, my dependent is healthy and in good physical shape, and I am not aware of any physical issues that would make them at risk if they engage in the program for which I am signing this waiver form.
I give YOU permission to use my words and to take pictures, or record a video of me or a dependent of mine. Additionally, I grant YOU the right to modify, make use of, and/or distribute the aforementioned items for commercial or non-commercial purposes, including online, and in any other media. Should a minor(s) accompany me at any time, I agree to be responsible for their behavior at all times while they are on the property and to supervise them in line with YOU's policies. If I do not want the minor in photos, I understand that it will be my responsibility to watch out for cameras and make sure YOU personnel know who not to photograph.
