*Registration is open for Pre-K (Must be Age 4 by June 1, and fully potty-trained) through the 5th Grade (entering 5th Fall 2023).

*Dates are Fridays in June: 2nd, 9th, 16th & 23rd- 9a.m. to Noon, with option for 1PM pick up/Lunch Bunch registrants

*You may Register for one, some of all of the dates.

FEES: This Event is FREE to Register. Optional 1pm pick-up/Lunch Bunch is only $5 per child, per day.

You will be given the option to fill in the total at the end of this form. Please add up the fees if they apply and enter in total in the box at the end of the form.

There will be option to pay at the door PayPal, Venmo, Cash or Check.

Scholarships available- Please reach out to Lexi at children@mcpcusa.org if you are in need of that assistance or have any other questions. We want to make sure that everyone who would like to participate can join in!

***OPTIONAL LUNCH BUNCH/1 PM PICK UP: We've added an option for Lunch Bunch/ 1PM pick-up with Lunch Provided for an additional fee of $5/child.

If you would like for your child to participate, please be sure to fill out that section and choose preferred meals (Option A/B/C) and specify any food restrictions/allergies in the allergy section.

DISCOUNT FOR FAMILIES/GROUPS OF 3+: You may register up to three children at one time. There is a payment cap on three children per family; however, if you would like to register more than three children, as a courtesy those additional registrations are free. You still must submit an additional registration & waiver for them, but fee is waived.

A Note on our Camp Volunteers: All Adult Volunteers are background checked through a third party agency and attend a safety training prior to working with the children.

I give my permission for Monument Community Presbyterian Church to use photographs and video footage of my child(ren) in newsletters, bulletins, and other presentations about the Church and its programs and people. All photographs and video footage shall remain the sole property of Monument Community Presbyterian Church I understand that no compensation will be made to me for this use.

From time to time, Monument Community Presbyterian Church will create web pages for publication on the Internet. All Web publications are subject to the following terms: all Web page documents will only include children’s first names; no confidential information regarding any child shall be published or linked to a Web page; and no names may be associated with pictures. I hereby grant permission for Monument Community Presbyterian Church to publish my child(ren)’s work, photograph(s), first name, or non-confidential information on the World Wide Web.

GENERAL, MEDICAL & TRANSPORTATION PERMISSION & RELEASE

As the parent or guardian registering my child(ren) I am voluntarily allowing my child(ren) to participate in Monument Community Presbyterian Church’s activities and programs, including transportation to/from off-site events. I waive, release, and agree to indemnify Monument Community Presbyterian Church and its volunteers and employees with regard to any claims, liabilities, expenses, or negligence of any kind arising out of my child(ren)’s participation in or travel to/from Monument Community Presbyterian Church's programs and activities.

In case of a medical emergency involving my child(ren), I understand every effort will be made to contact me. If I cannot be reached, I authorize a Monument Community Presbyterian Church representative to secure and consent to emergency medical treatment for my child(ren), and I agree to pay any costs incurred on behalf of my child(ren) for such treatment.

I also agree that basic, non-emergency, first aid may be administered to my child(ren) by on-site staff. A staff representative will inform me of any provided first-aid treatment.
