With this form, you can report any harassment or inappropriate behavior you have experienced or witnessed, or provide general feedback related to equality matters.

The responses will be visible to the guild's chairperson and equality officer. All matters will be handled confidentially. Nothing will be pursued further without the consent of the parties involved. Contact information of individuals will not be shared without permission.
