This form is required to cancel enrollment at World Elite Kids/Sweet Peas.
Policies to note before submitting:

for each class and each child.
Mid-month cancellation

non-refundable
but stays on account as a
credit
for future use.
No refunds
will be issued.
Avoid a $12 administrative fee
by submitting the form
by the last day of the month
(timestamp required).
