Do not use this form to request an early dismissal.  

This form is to be completed only by the student's legal parent/guardian. It is each student’s basic responsibility as a member of the school community to be on time and attend all classes. Daily telephone calls will be made to parents or guardians to confirm unreported absences. Absences and late arrivals will be recorded in attendance records. *Please note that Charlton Heights School administration reserves the right to contact you directly to verify the legitimacy of this form. Thank you for your cooperation.  

Homework can be picked up at the main office after 3:15 p.m.  **Please note: Per district policy, teachers are not required to provide homework for vacation absences.  Please email your child's teacher by 10:00 A.M. to request homework.

*Please note: Information will be shared with staff members on a need-to-know basis only.  Contact the nurse directly with information you do not want shared.
