DDMS Student Ambassador Application
Thank you for your interest in becoming a DDMS Student Ambassador! Ambassadors show leadership, good judgment, maturity, honesty, and integrity while upholding their responsibilities of being a student. Being an Ambassador is a tremendous honor, and with that honor comes various responsibilities.

If you are chosen as an Ambassador, you will have the opportunity to:
• Participate in community service projects and projects throughout the school
• Assist with Panther Pride Camp, Open House, Curriculum night, and other important events
• Assist with conducting tours for new students and other educators
• Serve as “buddies” to new students
• Help teachers and students during SMART Block
• Help out with other opportunities as they arise throughout the school year

As an Ambassador, you are a role model within Davis Drive Middle School. You will be expected to maintain a positive and respectful demeanor at all times toward students, teachers, and administrators. Failure to uphold your responsibilities and expectations of an Ambassador may result in your removal in the program.

Please complete this form as part of your application. Be sure to complete the Student Questionnaire as honestly as possible. You will also need two of your teachers to complete the teacher recommendation forms. Your teachers will give us the completed recommendation forms (you will not get them back). Your application will be considered in conjunction with your attendance rate, current grades, and discipline referral history.
