    Parent Plus Request Information and Instructions

    The Direct Plus Loan is available to help parents meet the cost of their child's college education. These loans are made to parents of full or half-time dependent undergraduate students. The amount borrowed may not exceed the cost of education less any other financial aid received. To apply for a Parent Plus Loan, the parent borrower must complete and sign this form. A credit check is performed on all applicants and the results are only good for 90 days.

    Authorization

    My signature affirms that I am the parent of the named student and that all the information on this form is true and correct. I consent to the U.S. Department of Education and its agents to obtain a report of my credit record and using the information from the report in determining whether to make a Direct Plus Loan to me. I understand that I will be notified in writing of the result of the credit check with respect to my loan application. I will use all loan funds received for educational-related expenses incurred at Methodist College UnityPoint Health. I also give Methodist College UnityPoint Health permission to issue any excess funds (after the student's bill is paid in full) directly to the student.
