2019 Capital City Classic Team WITHDRAWAL Form
This form is used for teams to notify us of withdrawal from the tournament.

Terms:
1) Teams withdrawing and requesting a refund before September 4th will receive a full refund.
2) Teams withdrawing and requesting a refund after September 4th and before October 5th will
receive a 50% refund.
3) Teams withdrawing on/after October 5th will not receive a refund.
4) Teams not checking in by 8:00 on the Saturday of the event will be dropped from the event and
will not receive a refund.
