I understand and agree to pay Chapter fee in full, 1 yr from start date and annually thereafter to maintain active status.

I understand Chapter fee is due in full immediately, if chapter dissolves within first year.

By submitting this application, the Signator understands and agrees to pay the Chapter fee one year from the date of this application, regardless of whether the Chapter decides to renew. The Signator also understands that with renewal, the Chapter fee must be paid annually thereafter to maintain Chapter status. Members and Chapters agree to all terms and conditions outlined in the "Law & Order" membership agreement of 40 Something Cowgirls. Furthermore, by submitting this application, all Members and Chapters agree to release all liability and hold harmless all parties involved, as stated in the 40 Something Cowgirls Liability Release Form. Chapter fees are paid annually to maintain membership in 40 Something Cowgirls and are used to support chapters through services and benefits, for the operation of Cowgirl Headquarters, and to sustain and grow our Cowgirl organization. *
