Welcome to Center for Campus Art's ' 62 Icons: Milestone in Furniture Design' exhibit appointment form.

Please read the complete guidelines below before filling up and submitting this form.

This form must be accomplished at least 2 working days before the date of visit. Bookings submitted after 3PM will be processed the next working day.

For guests that would like to schedule a Monday or Tuesday visit, names must be submitted by Friday, 12PM. Dates that are not included on the form are already closed to visit requests.

For guests entering campus IN THE SAME VEHICLE, kindly make sure that all passengers and/or drivers are declared in your booking form. CCA will not be liable for guests who will be denied entry because of incomplete booking details.

For those entering campus on foot, please file ONE (1) VISIT REQUEST PER GUEST.

Guidelines for Visit:
1. The Exhibit is open on Monday to Friday from 10AM to 6PM
3. For Benildean students, please make sure to have enough lead time when securing your gallery visit confirmation and student campus access requests.
4. Only fully vaccinated persons (kids and adults) will be allowed entry. NO VACCINE CARD, NO ENTRY.
5. Upon entry on campus, all visitors are required to accomplish a health declaration form as part of the university’s safety protocol. Please see this link for the form: https://apps1.benilde.edu.ph/hdf
6. Visitors are encouraged to monitor their health prior to their visit. Guests that have symptoms or have been exposed to someone with COVID-19 are encouraged to reschedule their visit to a different date.
7. Onsite guided tours are not available.
8. Large bags are not allowed inside the museum galleries. In the interest of time, guests are encouraged to leave large items in their respective vehicles. Otherwise, bags may be deposited at the baggage counter located at the ground floor.
9. Guests are required to present proof of vaccination and a valid ID at the lobby of the School of Design and Art Building.
10. Admission is free.
