Outstanding Youth in Philanthropy
This award recognizes an individual (under age 21) or youth organization with a majority of its members and/or leaders under age 21 whose philanthropic efforts (through fundraising and direct financial support, development of charitable programs, volunteering, and/or community leadership) demonstrates the spirit of philanthropy and sets an example for others to follow. The individual demonstrates an outstanding commitment to community service.

We recommend that you complete a draft of your nomination in a Word document and copy and paste this form. Nominations will be evaluated based on the quality and completeness of the responses to the questions below. Each member of the Awards Committee will objectively review and rank nominations and selections will be determined by majority vote.

Supplemental information (letters of support, photos, etc) may be submitted as part of this nomination and is addressed in the last question on this form.
