2026-2027 Daycare / Babysitter Transportation Form (Fill out one form per child)

This form is for parents to fill out if they are using daycare or babysitters instead of their home stop. The babysitter must be in the regular bus stop area of the home school. The daycare must be located in Bayville. Please note: this changes the busing Monday through Friday only. There will be no sporadic stops. Please note the change will not take effect for 2 working days after submission. See below for list of approved Daycare stops.
