I waive and release Mo’s Soccer Camp, its directors, coaches, staff, independent contractors, and FC Columbus from all liability, claims, demands, and causes of action arising out of or related to any loss, personal injury (including death), disease, illness, or property loss that may be sustained or occur from participation in Mo’s Soccer Camp.
I completely understand all the risks associated with participation in this program. I certify that the undersigned child is physically capable of participating in the Mo’s Soccer Camp and all related activities without any restrictions.
I understand that any personal items lost or stolen will not be replaced by the Mo’s Soccer Camp.
I understand that my child may be photographed/videotaped during the camp. I agree to allow my child photo/video to be used for any legitimate purpose by the Mo’s Soccer Camp.
I have read and agree to the cancellation policy as stated on the camp website. I also understand there is no refund if your child must leave camp early due to illness, injury, breaking of camp rules, homesick, or simply dissatisfied with experience.
I will not send my child to camp with any sickness. I will cancel the registration and agree to the cancellation policy.
I agree to all Covid-19 related camp policies, waivers, and camp rules. Failure to follow these policies / rules may result in registration cancelled or child sent home immediately.
