Cat Adoption Team Spay/Neuter Appointment Request Form
Our low-cost spay/neuter program aims to assist low-income families and their cats by offering affordable spay/neuter surgeries for those who qualify. There are three pricing levels depending on qualification. Please enter your information if you own a cat that needs to be spayed or neutered. We'll follow up with you about appointment availability when we have an opening for your cat/s. You only need to complete this form one time.

To qualify for our spay/neuter program, you must live in Clackamas County, Multnomah County, Washington County, Clark County, OR live within one hour of our Sherwood shelter

In order to request an appointment for spay/neuter you'll need to answer yes to the following questions. Please call us if you have additional questions about these requirements.
The cat/s I'd like to get fixed are my personal, domestic cats. For information about fixing feral cats please contact the Feral Cat Coalition of Oregon at (503) 797-2606.
I have a carrier for my cat/s or can obtain one. Cardboard carriers can be purchased in advance at CAT for $8. If cost is an issue please contact CAT prior to your appointment at (503) 925-8903.
