ISO WALTER HOGAN CONFERENCE CENTER: MEETING AND FUNCTION ROOM
GUIDELINES

ISO ensures that:
 All participants are fully-vaccinated.
 Body temperature will be checked at the entrance of the building. No need for checking
body temperature at Alert Level 1.
 Blue Pass QR codes of regular visitors will be scanned every
time they enter the premises. All participants, however, are required to accomplish this CAR form.
 Rooms are sanitized and cleaned before and after use
 Adequate ventilation is practiced
 Alcohol dispensers are placed at strategic areas
 Signage posted are visible and informative
 Markers on the floor are placed to maintain the physical distancing of each table and
chairs
 There is a designated isolation area inside the building, located on each floor of the
building (beside the stairwell)
 The physical distancing of a 1.5-meter radius was maintained between individuals at all
times
 Face-to-face interactions between individuals are allowed no more than 30 minutes.
 Segregation of waste (Biodegradable and Non-Biodegradable, Infectious waste bins
for surgical mask, tissue, and wipes) are practiced and monitored
 Restrooms are cleaned and sanitized regularly

Face-to-face meetings are an essential part of business and academic life. Find out here
how you can conduct your meeting and conferences in our function rooms while following the
Health and Safety Guidelines:
1. Meeting participants are required to follow the minimum health standards:
a. Wear a face mask in the building
b. Sanitize your hands immediately upon entry into the conference rooms.
c. Avoid touching your eyes, nose, face, and mouth with unwashed hands.
d. Practice physical distancing of 1.5-meter radius at all times.
e. Refrain from shaking hands with other people.
f. Cover your mouth when coughing and sneezing
g. Frequently wash your hands. Use only paper towels to dry hands

2. Strictly follow the occupancy limits. Do not exceed the posted capacity of the meeting
room, seating must be available for all attendees and spread out.
3. Do not move or rearrange the tables. Always look for the markers on the floor. This will
limit the number of people touching the tables before and after meetings. This also
complies with the 1.5-meter physical distancing recommendations.
4. Please refrain from going into other rooms. The only room designated for your meeting is the
reserved conference room and the restrooms.
5. Mingling with participants of other rooms is not encouraged unless required by the
facilitator. Limit face-to-face interactions to less than 30 minutes if necessary.
6. Avoid sharing things. If necessary, make sure to sanitize it before and after borrowing.
7. Please limit the usage of the restroom to 2 people at a time unless you are assisting with
care.
8. Sanitize surfaces.
9. Throw your garbage at the designated waste bins. (Biodegradable, Non-biodegradable
and Infectious waste bins)
10. Avoid foods and drinks in meeting rooms. Eating inside is strictly prohibited. Outdoor
eating is highly encouraged.
11. Follow CLEAN AS YOU GO
a. If you use it, you clean it.
b. Do not leave used containers, mugs, coffee cups, etc. in the meeting room. Take
them home or dispose of them in the garbage/recycling.
c. Wash your hands or use hand sanitizer after cleaning your area.

We request everyone to take responsibility for the safety and wellness of everyone by
refraining from going to ISO premises if you are experiencing a fever, cough colds, or any of
the other symptoms of possible COVID-19 infection. In such a situation, you can immediately
get in touch with the ISO management to properly assess and manage the situation. Here are tips on how to handle COVID-19 situations during the event:

How to handle yourself if you are sick:
a. Kindly inform the facilitator before the event if you are not feeling well or having COVID-19 like symptoms. Refrain from coming to the venue.
b. If you feel sick during the event, please notify the facilitator immediately to receive proper
assessment and management.
c. Make sure to always wear your facemask and sanitize your hands.

How to handle sick and unwell participants as a Facilitator:
a. Make sure that the participants will not attend the event if feeling unwell or have COVID-19 like symptoms.
b. If ever the symptoms appear during the event, immediately notify ISO management.
c. Escort the participant to the Isolation area and wait for the Safety officer or Medical
personnel to conduct proper assessment and management.

The above protocols and regulations were based on the ADMU and IATF guidelines which
are required by the ADMU, IATF and also CHED, DOLE, and the LGU on which we operate. So,
all are expected to follow the ISO protocols to help reduce the risks of exposure and infection.

IMPORTANT REMINDERS:

1. All participants should fill out this form. CAR forms may be submitted between (indicate time schedule, if any). Please wait to be notified by ISO of your approval.

2. Each person who wishes to attend this event should fill out one CAR form.

3. Only fully-vaccinated persons (minors and adults) are allowed to enter the campus. Upload your vaccination card (front and back) in this google form is required.

4. Interested participants who are sick or not feeling well are strongly advised not to enter the campus.
 
5. Everyone should wear the prescribed face mask (face shield is optional). NO FACE MASK NO ENTRY.

INFORMATION ON SARS-CoV-2/COVID-19, ASSOCIATED RISKS, AND HEALTH PROTOCOLS

SARS-CoV-2/COVID-19 is a highly contagious virus/disease. Contracting it may result in mild to serious illness, serious health consequences, or even death.

The DOH, IATF, and other public health authorities have issued regulations to reduce the spread of the virus. Accordingly, the University has put in place the necessary preventative measures as directed by the government, including, but not limited to:
1. Screening, detection, containment, contact tracing, and lockdown protocols;
2. Emergency transfer, quarantine, and isolation protocols;
3. Physical distancing protocols;
4. Maintenance, sanitation, and disinfection protocols;
5. Cyclical student and personnel shifting plan; and
6. Engineering controls to ensure physical distancing, ventilation, physical hygiene, and environmental hygiene.

While the University has taken these measures and will ensure compliance with subsequent regulations that the government may issue, it cannot guarantee that persons who go on campus will not be exposed to or become infected with the SARS-CoV-2/COVID-19 or that they will be spared of the potential consequences of exposure or infection. Exposure to or getting infected with the SARS-CoV-2/COVID-19 may be a result of one’s own actions, omissions, or negligence, and that of others, including University stakeholders, family members, guests, etc.

All are expected to adhere to public health measures and University protocols to help reduce the risks of exposure and infection.

b) I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, muscle pain, and sore throat.
