20-21 Clark County Public Schools Enrollment
Please complete the below information to register your student for the 2020-2021 School Year. If you are enrolling multiple students, please submit one form per student. Once the registration is completed your student(s) will be enrolled into Infinite Campus our student information system. You will then be contacted by email as to who you will need to send the required documents to. You can send the documents electronically by scanning them or by taking a photograph with your cellular device.
Required Documents for Registration:
Student's Birth Certificate
Up-to-date KY Immunization Certificate
Two Proofs of Residence, such as utility bill, tax bill/tax receipt, lease agreement, property deed, sale agreement.
Other Registration Documents that will need to be sent prior to August 12, 2020:
Social Security Card
Proof of a preventative health care exam
Proof of an eye exam (After January 1, 2020)
Proof of a dental exam (After January 1, 2020)
Never submit passwords through Google Forms.
