SOAR Cohort Sign-Up
SSI/SSDI Outreach, Access, and Recovery (SOAR) is a program designed to increase access to SSI/SSDI for eligible adults who are experiencing or at risk of homelessness and have a mental illness, medical impairment, and/or a co-occurring substance use disorder. The self-guided SOAR online training takes case managers step-by-step through the SSI/SSDI application process. Sign up to join other Lake County case managers in this important training.

The cohort will work on two or three classes independently each week, and join in a conference call each Friday to receive technical assistance with the training. 20 CEUs from the NASW are available for those who successfully complete the course.

The training will last from October 10th thru November 3rd. Weekly cohort conference calls will take place on Fridays at 11:00am. You can expect to receive an email welcoming you to the cohort and calendar invites for the upcoming calls. Thank you for your interest.
