2023-2024 ConnCAT After School Application
ConnCAT welcomes 5th through12th graders as they begin their afterschool venture for the 2023-2024 School year!!

Parents, please fill out the application CAREFULLY and COMPLETELY.

Note: Completion of the application does not guarantee  acceptance into the  program. 

PLEASE DO NOT REMIT ANY MONIES UNTIL YOU ARE NOTIFIED OF ACCEPTANCE!

DETAILS
-    MASKS are optional unless new guidelines are given. Families will be updated as needed.
-    Program Dates: October 10, 2023- June 2024
-    Hours: Mon - Fri 2:00pm-6:00pm

-     DO NOT SUBMIT ANY DOCUMENTS UNLESS YOU ARE NOTIFIED OF YOUR CHILD'S ADMITTANCE TO THE           PROGRAM

APPLICATION GUIDELINES:
-    Parents/Guardians must submit one (1) completed application. Incomplete applications will be waitlisted.
-    Be certain that you want your child to participate in the program.
-    We understand that children may be involved in extracurricular activities; please inform us if this applies to          your child.    
-     Please ENSURE THAT THE APPLICATION IS ACCURATE.  Inaccurate information may cause your child not           to be admitted.
-    Please provide us with ACTIVE & ACCURATE phone numbers and email addresses/emergency contacts.
-    Please click on "SUBMIT" at the very end of the online form to ensure delivery.

FALL 2023 Rolling Admission:

~APPLICATION OPENING DATE:  AUGUST 25, 2023-SEPTEMBER 15, 2023

*AFTER SCHOOL BEGINS: OCTOBER 10, 2023

~ROLLING ADMISSION:  OCTOBER 20th -NOVEMBER 10. 2023

SPRING 2024

~APPLICATION OPENING DATE: JANUARY 5, 2024-JANUARY 19,2024

~ROLLING ADMISSION: FEBRUARY 2, 2024-MARCH 15, 2024

Parents will be notified via email of their acceptance.    
Applications received after deadline will be accepted on a rolling admission.
