Writing Center Workshop - Attendance Form
In order to receive a verification form for the workshop, you must sign-in and sign-out using this Sign-in Form.  Please note that students must attend the entire duration of the workshop in order to receive a verification email.

Be sure to do the following:

1. Select the topic of the workshop that you are attending.

2. Enter the date of the workshop that you are attending.

3. To Sign-in for the workshop, enter the following information:
  • Enter the time that you entered the workshop.
  • Enter your full name.
  • Enter your student ID number.
  • Enter your SBVC email address.
4. To Sign-out of the workshop, enter the following information:
  • Enter the time that you leave the workshop.
  • Enter your full name.
  • Enter your student ID number.
  • Enter your SBVC email address.

Note:  Students who attend the Writing Center workshops may be enrolled in the LST-606 noncredit course.  Students will not be charged for the course, nor will it affect their GPA.  It is used to document usage of services to help with apportionment funding for tutoring/learning services.  For more information, visit:  https://www.valleycollege.edu/student-services/tutoring-academic-support/writing-center/lst-606.php
