Frontline Records Request 
Please complete this form in order to request records through Frontline.  We can only request from districts that are currently Frontline users. The request needs to be the most recent school district the student attended. You will receive an email once the file is received or if the record cannot be found. Once the record is received, please change the status to "Currently Served" and put in the correct local ID (please, do not leave the TR##### ID). Please correct the PEIMs errors. Please do not add a student to Frontline manually prior to receiving the records as this will create two records for the student. You will receive an email  once the request is transferred and visible.
