Ashford Medical Partnership Patient Survey
The Ashford Medical Partnership Patient Survey is a great way for the practice to know exactly how our patients feel about the service we provide. Your feedback will help us to identify any problems that need addressing and other ways to improve the service for you and all our other patients.

This survey is anonymous so please be completely honest when answering the questions.

The survey will cover the topics of your experience with the clinicians, the reception team, appointment access, waiting times, prescriptions, COVID-19 and overall care. If there are any topics or questions that do not apply to you then please skip the question or use the "does not apply" option in the questions. This survey will take you roughly 5 minutes to complete.

This survey will close on Monday 31st May 2021. The results will be published in June for the hope of repeated this survey in December 2021.
