Point of Contact (PoC) Full Name
This is the person who will act as an ambassador for the school or organization and handle all scheduling, coordination, and payment regarding the Arts in Education (AIE) Program.

Available Dates
For performances, please list 2-4 preferred dates for each Performing Artist.
For in school field trips, please list 3-5 dates that work for you (ex. March 11th, April 17th, May 4th).
For artist residencies, please list 2-4 date ranges of 5 days that work for you. Ranges are best when listed as consecutive days in a week (ex. Monday-Friday, March 7-11).

All offerings run between 45-60 minutes for a single performance or workshop. Please indicate which times during the day you want to offer these experiences.

PAYMENT AND CANCELLATION POLICY
Contracts and invoices will be sent to the school upon scheduling confirmation. Full payment is due within 1 week of program completion.

Any site wishing to cancel programming must do so at least 2 weeks prior to start date. Cancellations less than 2 week prior to program start date will be charged the full contracted amount.
Any cancellation occurring after August 31st for fall programming or after January 1st for spring programming are subject to a cancellation fee, 20% of the contracted amount.
