Church of St. Anne Faith Formation COVID Pre Screening
When attending any catechetical/faith formation session(s), families are attesting that they are free for COVID symptoms, have not been diagnosed and/or exposed and/or suspected exposure to COVID within the last 14 days, and have not have been in close contact with anyone that traveled on a cruise ship or
internationally or to an area with a known communicable disease outbreak in the past 14 days?
This is a one-time form until such time we are instructed to ask for more frequently.

Children, volunteers and staff who are exhibiting any signs of illness must not attend catechetical sessions.
Children, volunteers and staff will be asked to leave or not come to class if they test positive for COVID-19 or exhibit one or more of the symptoms of COVID-19, based on the CDC’s current list of symptoms.
Should a child, volunteer or staff become symptomatic during a catechetical session, they will be isolated from others and parents will be notified to pick up the child immediately. Children, volunteers and staff with symptoms related to COVID-19 will be safely and respectfully isolated from others. There will be an isolation area in the school.

Results must be documented when signs/symptoms of COVID-19 are observed. The Church of St. Anne is obligated to notify the Board of Health of any possible/reported cases of COVID.

Everyone attending catechetical/faith formation sessions in the school , church or any church building are required to wear a mask covering their nose and mouth.
For children attending Faith Formation sessions in the school, parents can walk children to the outside school door, however parents/adults are not able to enter the school proper. For sessions in the Church parents/adults should attend and required to wear a mask regardless of vaccine status.
