Student Sign-in/out Form
Parents/Guardians must use this form to grant permission for their student to leave the school day early, or arrive to school late. Once the parent/guardian arrives at the school, please call the office at 218.378.4134 for dismissal. Parents will no longer be required to sign their student out in the office, as long as this form has been completed. The information provided will be sent to the school office for attendance.
